If you have downloaded the level cap increase DLC instead of Playthrough 2.5 you will have the choice to enter Ultimate Vault Hunter Mode (UVHM, Playthrough 3)Ĭomplete the main story. The DLC can be played Normal Mode (Playthrough 1), True Vault Hunter Mode (TVHM, Playthrough 2) and Playthrough 2.5 as you can in the main game. With recent hot fixes it may or may not be beneficial to play signed onto the PSN. You access this DLC by fast travelling to Badass Crater of Badassitude via a Fast Travel Station. You should not have to complete any of the "Tier 3 Rematch" missions, but if you activate a single rematch you may have to complete it and others to get the trophy I will not list them in the mission guide as they are in the same location as the previous ones and to avoid activating a quest that that could make it take longer to 100%.

The Motorhead trophy is not glitched per say, but it is worth mentioning that a few people have different concepts of how to obtain it.
I'm not completely sure why but you have beaten the main game and it is saved in your profile data, then the Motorhead trophy will have a few missions that recommend you be level 49 (in Playthrough 1). The trophies for this DLC are straightforward.
